Continuing Interprofessional Development and Program Evaluation

PROGRAM EVALUATION:

A multifaceted program evaluation is critical to enable program evolution and support for a sustainable LBP model of care.

Evaluation needs to reflect relevant local and national key PATIENT, PROVIDER, and SYSTEM metrics.

  • Patient: Satisfaction, Outcomes, Accountability re goal attainment (e.g. tools: start-back, ODI)
  • Provider: Satisfaction, KT, System support (Other to be confirmed)
  • System: Access times for different providers, Cost avoidance, ER visit, referrals
    • This will require linkages with clinical and administrative stakeholders
  • National Framework for a Patient Centered LPB Model of Care
  • Timely Access to Primary and Specialized LBP Care
  • Evidence-Based Primary and Appropriate Specialist Assessment, Investigation and Management
  • Inter-Professional Shared-Care Approach
  • Continuing Inter-Professional Development
  • Long-Term Self-Management Strategies, Chronicity Prevention and Health Promotion Strategies
  • Program Evaluation
