These tools have been developed from the 2010 Clinical Practice Guidelines for the Diagnosis and Management of Osteoporosis in Canada.
Letter to physicians
The assessment for and management of osteoporosis for the majority of patients takes place through the primary care physician. It is therefore extremely important the primary care physician is made aware of any fracture, osteoporosis testing or treatment that is initiated. The attached letter is an example of a letter that can be used to provide the relevant information to the physician.
Letter to patient and family
Many patients who have sustained a hip fracture also present with osteoporosis which require appropriate testing and long term medical maangement. The attached letter is an example of a letter which can be provided to patients and families to provide them with information on osteoporosis and the medical management.
